Web page linking help needed (Code Fix)

My business has a web page that is malfunctioning. Need help to correct the script on 2 web pages to correct links that are broken.

The problem is on a form filling for an on line reservation, and credit card update.

Should take no more than 30 minutes to one hour to complete for web page designers. Need help ASAP please!
